Documentation of Medication by History within the electronic medical record has been an ongoing issue for the Nursing staff. Unfortunately, if the Medication by History is not documented correctly, it has a direct impact on the Medication Reconciliation conversions when completed by Physicians, which then effects patient's Discharge Instructions at time of discharge. The details of the documentation must be displayed a certain way for the medication to convert correctly. There has been ongoing education through additional classes, and job aids. I created this infographic as a simple and to the point visual in order to remind Nursing how their details should display within the EMR. This infographic assignment could be sent to Leadership at the Health Quest facilities and displayed on each Unit's information board or anywhere considered to be "high traffic" areas for Nursing.
